Personal Details

All details must be completed by an Adult even if the registration includes a Senior participant in the collaborative choir. Please be aware that any Senior participants in the collaborative choir must be accompanied by an Adult on the day. Our next pop-up takes place in Town Square on Saturday, 22 February 2025 (10am to 2pm). Sheet music, backing tracks, vocal group rehearsal tracks and details for the workshop will be provided to all participants the week prior to the workshop.

I confirm that I have downloaded and read through the waiver regarding my participation in The Collab Collective.

I confirm that I am aware of the no refund policy being in place due to Collab Company making purchases and bookings on my behalf ahead of the workshop.

I confirm that I am permitting Collab Company to give my place to a participant in their Pay It Forward program should I be unable to attend.

I confirm that all details provided for the purchase of wearable merchandise are accurate.

I confirm that my reservation will be held for 3 days for payment to clear, but will be deleted off the system thereafter if payment is not made and I will have to begin the booking process again.

I understand that there is a capacity of 100 participants due to the venue we are popping up in.

I will print out and hand my Medical History and Guidance Form to a member of Collab Company on the workshop day should this be required.

I agree to all terms and conditions which have been stipulated in the waiver and within this registration form.
